how do you teleconference on a mac?? instant video messaging…?

i see it in movies where you can talk to someone and see their picture, live video, on a apple notebook. i know i could just call the mac people but i was wondering if anyone here has used it ever and is familiar with how it works also who ever has would u mind walking me through the set up maybe even a live demo for me?
